Slicer failed on Samsung SIR-4080

I downloaded and ran slicer 1.9 on my hacked Samsung SIR-4080. I had version 6.2 running and I was upgrading to 6.4a. The script ran fine and everything looked good until the unit rebooted. I ran in to a problem, here is the section of the log I believe is the problem:

Running Slicer hosed my install so I ended up restoring the drive using mfstools using an image from one of my other tivos. After I got this all up and running, I ran slicer again. I ended up with the same exact problem... slicer seemed to run fine, but when rebooting in to 6.4a, I ended up with this error.

I am wondering if maybe it has to do with a patched tivoapp? It was so long ago that I hacked these units, I don't remember exactly what I did (I did not hack with zipper). But it looks like it is the tivoapp that is crashing... any thoughts?
